Safety and efficacy of intra-articular anti-tumor necrosis factor α agents compared to corticosteroids in a treat-to-target strategy in patients with…

2015

The aim of this study was to assess safety and efficacy of ultrasonography (US)-guided intra-articular injections using tumor necrosis factor (TNF) blockers compared to corticosteroids in rheumatoid arthritis (RA) or psoriatic arthritis (PsA) patients, experiencing refractory monoarthritis despite the current systemic therapy. Eighty-two patients were randomized to receive three intra-articular injections monthly of either corticosteroid or TNF blockers. Primary endpoints were the safety and an improvement greater than 20% for visual analogic scales of involved joint pain in patients injected with anti-TNFα. Further clinical, US, and magnetic resonance imaging (MRI) evaluations were consid…

Therapeutic synergism between hyaluronic acid and dexamethasone in the intra-articular treatment of osteoarthritis of the knee: a preliminary open st…

1992

In vitro studies on the effects of dexamethasone on human synovial cells have shown that with high concentrations of the steroid in the culture medium cellular activity was completely blocked whereas with low concentrations (10(-6)M), cellular density decreased but there was an increase in the synthesis of RNA, DNA, protein and hyaluronic acid. These data, coupled with clinical experience of using intra-articular hyaluronic acid to treat patients with osteoarthritis of the knee, prompted the investigators to carry out an open, randomized study of the use of very small doses of dexamethasone in association with hyaluronic acid in 40 osteoarthritic patients. Twenty patients received a weekly …

Intra-articular hyaluronic acid injections for hip osteoarthritis

2018

Although viscosupplementation has been used in the past few years both for knee and hip osteoarthritis (OA), the number of intra-articular injections and the interval between doses still remains an undetermined subject. The aim of this open retrospective study was to evaluate the clinical and functional outcome in patients with mild-moderate hip OA treated with a course of 1, 2 or 3 Hyaluronic Acid (HA) intra-articular injections. Ninety-six patients were included: 19 patients received only one injection, 24 received two injections, and 44 received three injections. Age, sex, VAS for pain and WOMAC score before each intra-articular injection, number of intra-articular injections, reasons fo…

Potential Use of a Diluted High-relaxivity Gadolinium-based Intra-articular Contrast Agent for Magnetic Resonance Arthrography: An In-vitro Study

2019

Abstract Background Magnetic resonance arthrography (MRA) requires intra-articular injection of gadolinium-based diluted paramagnetic contrast material. To our knowledge, gadobenate dimeglumine (Gd-BOPTA) has never been used for intra-articular applications. Our aim was to test in vitro different concentrations of Gd-BOPTA to be potentially used to perform MRA. Methods Gd-BOPTA was diluted in saline (NaCl 0.9%) to achieve different concentrations (4 mmol/l; 2 mmol/l; 1 mmol/l; 0.67 mmol/l; 0.5 mmol/l). Six sets of five sterile pipes were prepared with 5 ml of each solution, five sets added with 0.5 ml of fresh synovial fluid. Two separate pipes were prepared with 5 ml of gadopentetate dimeg…

A comparison of the effects of Methylprednisolone Acetate, Sodium Hyaluronate and Tenoxicam in the treatment of non-reducing disc displacement of the…

2018

Background This clinical study aimed to radiologically and clinically compare the effect of intra-articular injection of methylprednisolone, sodium hyaluronate or tenoxicam following arthrocentesis with that of arthrocentesis alone in patients with non-reducing disc displacement. Material and Methods A total of 44 patients radiographically diagnosed with non-reducing disc displacement of the temporomandibular joint (TMJ) were randomly divided into four treatment groups, as follows: Group 1, arthrocentesis alone; Group 2, arthrocentesis plus methylprednisolone acetate; Group 3, arthrocentesis plus sodium hyaluronate; Group 4, arthrocentesis plus tenoxicam. Maximum mouth opening (MMO), latera…

The comparison of knee osteoarthritis treatment with single-dose bone marrow-derived mononuclear cells vs. hyaluronic acid injections

2017

Objective: The aim of this study was to compare treatment methods of the knee joint degenerative osteoarthritis, using autologous bone marrow-derived mononuclear cells and hyaluronic acid injections and observe prevalence of adverse effects in both groups. Materials and methods: A prospective randomized controlled clinical trial was carried out. The analysis of pain and changes in osteoarthritis symptoms after a single intra-articular bone marrow-derived mononuclear cell injection into the knee joint in the Kellgren– Lawrence stage II–III osteoarthritis during the 12-month period were performed. The results were compared with the control group treated routinely by hyaluronic acid injections…

Interventistica eco-guidata per l’articolazione d’anca

2013

The purpose of this work is to review the main applications of ultrasound-guided hip intervention al procedures describing the indications and methodology. Ultrasound is a method considered safe for patients as it has not been shown a possible damage due to ultrasound energy transmitted towards the patient's tissues as opposed to methods that make use of ionizing radiation such as conventional radiography and computed tomography. In addition ultrasound is quick and effective in guiding the needle to the desired location avoiding vascular and/or nerve structures thus enabling greater security compared to the techniques performed in interventional fluoroscopy which also have the disadvantage of a large delivery of ionizing radiation for both patient and operator. Several authors have described the main techniques of ultrasound-guided hip intervention and its various applications that can be both diagnostic (as in the case of joint fluid aspiration to assess the presence or absence of septic arthritis) and therapeutic (as in the case of drugs inoculations or evacuation of exceeding intra-articular fluid).

Which route of administration of acid tranexamic, intravenous or intra-articular, is more effective in the control of post-surgical bleeding after a …

2019

Abstract Introduction One of the most frequent complications after a total hip arthroplasty (THA) is bleeding, intravenous tranexamic acid (TXA) is used to reduce it. We considered it necessary to carry out a study to clarify which administration route is superior. Material and method Prospective, controlled and randomized study in 2 arms carried out between February 2017 and February 2018. 15 mg/kg of intravenous TXA were administered in group A and 2 g of intra-articular TXA in group B. The values of haemoglobin and haematocrit were evaluated at 24 h-72 h, blood loss volume, drained blood volume, transfusions and complications. Results 78 patients were included, 31 with intravenous treatm…
